The day of your first visit, please arrive at least10 minutes before your scheduled appointment. You'll need to bring your current insurance card with you. Please note that your payment will be due at the time of your appointment
Returning patients
Please arrive at least 5 minutes before your scheduled appointment. Be sure to bring your current insurance card with you to each appointment. Please notify the front desk of any insurance, phone number or address changes.
Please call the office in advance, when possible. If you do not contact us regarding a cancellation, we charge a cancellation fee of $25.
Many patients come to the General Practitioner's office with the infamous "list" of problems. It is neither practical nor in the best interest of your health to cover a variety of issues and make quick diagnoses in one appointment. What seems like one simple symptom to a patient, for example--abdominal pain, often leads to lengthy questioning, exams and the ordering of tests. Therefore, we ask that you PRIORITIZE your problems and be prepared with the names of your medications if you need refills. Dr. Shu will let you know what can be covered at the start of your visit. Follow up appointments will have to be made to cover other issues. Also, please recognize that a physical appointment is just a preventive physical exam. This is not the time to discuss other health matters, unless it relates directly to your physical such as an abnormal lab finding.

Dr. Shu is an office procedure expert, called a proceduralist. He provides many specialty cares by doing numerous office procedures. Please see the procedure list in the services section of the web site.
Dr. Shu performs the high quality procedures in his office. Our patients benefit from having procedures performed in the familiar, and often less-expensive, environment. In contrast to procedures done in an inpatient setting, there is no facility fee for office procedures. Continuity of care is enhanced. Our patients also benefit from earlier diagnosis and treatments and using their precious time more efficiently.

For emergencies call 911, in cases of medical urgencies, go directly to the appropriate urgent care or emergency room. If it is not medically urgent, please call 952-922-9999, and follow the directions.
It is our policy to fill routine prescriptions during scheduled office visits. It is our intention to provide you with ample medication until you need to return for your next visit. Please contact your pharmacist for refill, allow 48 to 72 hours for refill request. If you are out of these medications it is likely that it is time for you to be seen and you will need to make an appointment. Refill requests by telephone or fax to One Stop Medical Center will not be accepted. Please check with your pharmacy regarding the status of your refills.
There are many types of infections that need various types of treatments. Diagnosing and treating patients over the phone does not give you the high level of care you and your family deserve. We will be happy to work you into our schedule right away so you can have timely, appropriate medical care. Antibiotics that are not taken properly or are taken for non-bacterial infections lead to antibiotic resistance and "superbugs" that are becoming a serious medical problem in this country.
Some insurance plans require that specialist referrals be provided by a primary care physician. Our policy is that an office visit is required for all new referrals to a physician or for medical conditions for which you have not been seen in our office.. This policy is to familiarize your primary care physician with your medical condition and to improve communication between your PCP and specialist. Our referral coordinator will be glad to answer your questions by phone or e-mail. Retroactive referral authorizations will not be made.
We don't accept other records until you are an established patient. We PREFER that you release the records to YOURSELF and bring them in. Often we don't need the entire chart, and we will make copies of the pertinent information necessary to complete your medical record.
Send a signed release for medical records with an address or fax number along with a $25 processing fee and we will take care of the transfer.
Signed authorization is required for all requests to transfer medical records. This can be accomplished:
Procedure Clinic participates with most major insurance companies. If you have medical insurance, please provide that information to our staff when you check in for your appointment. We will be happy to file claims with your insurance company on your behalf.

Based on the product and member's benefit levels the requirements to see those members will differ. Please call the member services # on the back of your insurance card to confirm coverage and requirements, or check their websites. Ask about to what type of visits deductibles apply and about co-payment amounts.
Many patients don't realize that billing is a courtesy and not a requirement of medical offices. Ultimately, the patient is responsible for the fees and this is stated in a form which you sign at registration. If there is a problem getting the claim paid after several attempts on our end, we allow patients to investigate the claim further before forwarding the bill to them. Patients can learn about the quality of customer service provided by their insurance company regarding these matters. This is helpful in making decisions about which plan to choose in the future. Hold times can be over 20 minutes, and we have neither the staff time nor financial resources to do this for multiple patients. One hint is that when you finish giving all your information to the automated system, simply say, "customer service" or "operator." The automated claims info never gives you the level of detailed explanation we need.
We offer 20% discount to self payers, the self-pay patients also get low lab fees through Fairview lab. Please be aware that you will be required to pay for your initial office visit at that time.
All labs and X-ray are conveniently done on the next floors of the same building.
We usually get the faxed results in a few days. Some results take longer depending on what was ordered. The biopsy results take one week. X-ray preliminary reports are ready at the same day. No news is good news, but if you want to hear one way or another, contact the office. Minor abnormalities are discussed at the follow-up appointment.
We are happy to provide you with whatever documents you need. Either drop by the office anytime we're open, or call us and provide us with a secure fax # or we can mail to you.
Payment is expected at the time of service; we accept cash, checks, VISA, Master Card. Most patients have insurance and payments are obtained by billing the contracted insurance. Appropriate patient portions are collected at each visit.
If your problem is straightforward and you do not wish to come in to the office, you may go to www.shuMD.com for an eVisit. You must be an established patient for eVisit. There is a small amount fee for each eVisit.
We follow all federal HIPAA guidelines for confidentiality of your health information. All patients are given a copy of these policies. If you need another copy, please ask our office staff.
